Initial Comments:
An offsite follow-up survey completed January 27, 2022 found that Lehigh Valley Therapy, Inc. had corrected the deficiencies cited under the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a Medicare recertification survey conducted on November 4, 2021.

The deficiencies were cited as a result of a Medicare recertification survey conducted on November 4, 2021.
